Nominate a Paralympic Torch bearer before 22 November 2011

London 2012 are looking for 580 inspirational torchbearers working in teams of five to carry the Paralympic Flame from Stoke Mandeville to Stratford over a 24-hour period on 28 August 2012.
Nominees must be over 12 years of age. Disabled and non-disabled people from all corners of the UK are invited to take part.
Nominations close at 5.59pm on 22 November.
